 Acknowledgment of Unstaffed Access

I understand that I am requesting 24/7 access to the facility, including during unstaffed hours. I acknowledge that there may be no personnel on-site during the times I choose to use the facility. I understand and accept full responsibility for my safety while using the gym during unstaffed hours.

Assumption of Risk & Release of Liability

Physical activity, equipment use, and fitness classes involve inherent risk, including injury or death. By signing this form I Acknowledge these risks- Voluntarily accept them- Release Probodies, its owners, staff, and affiliates from all liability I agree to assume all risks and waive all liability associated with gym usage.

24/7 Access Rules Agreement

I agree to follow all rules including (but not limited to):- No sharing of access key or entry code- No guests during unstaffed hours- No drugs, alcohol, or unsafe conduct- Use equipment responsibly- Do not use the gym if unwell or under medical restrictions- Call 999in case of emergency I agree to follow all rules and understand that violations may result in revoked access without refund.

Video Surveillance I understand the facility is monitored 24/7 by security cameras for safety and liability. My presence and activity may be recorded. I consent to video surveillance during gym use.

KEY FOB

Use your own key fob and key code every time you visit Probodies,as it is personal to you, please do not let anyone else use it or Probodies will cancel your membership. We monitor your key fob usage by CCTV 24/7 to ensure the safety of our members, and misuse may resuly in us applying additional charges to you membership fees. Any misuse will result in being fined £100.00 or you membership cancelled.

Health Declaration

I certify that I am in good physical condition and able to safely use gym equipment. I will not use the gym if unfit to do so.  I acknowledge I have read, understood, and agree to all of the above terms.
